1. Services Provided
PrecisionPoint Diagnostics LLC provides mobile diagnostic testing services, including but not limited to drug and alcohol testing, specimen collection, and laboratory coordination. All services are performed in accordance with applicable federal and state regulations.
2. Eligibility for Services
Clients must be at least 18 years of age or have legal guardian consent. Valid identification is required at the time of service.
3. Appointment Scheduling & Cancellations
Appointments must be scheduled in advance. A minimum of 24 hours’ notice is required for cancellations or rescheduling. Failure to provide proper notice may result in forfeiture of fees.
4. Payment Terms
All services must be paid in full at the time of booking unless otherwise agreed upon in writing. We accept approved payment methods as listed on our website.
5. No Guarantee of Results
PrecisionPoint Diagnostics LLC does not guarantee any specific test results. All laboratory outcomes are determined by certified labs and are outside of our control.
6. Client Responsibilities
Clients are responsible for providing accurate and complete information. Any errors in submitted information may impact test results and are not the responsibility of PrecisionPoint Diagnostics LLC.
7. Limitation of Liability
To the fullest extent permitted by law, PrecisionPoint Diagnostics LLC shall not be liable for any indirect, incidental, or consequential damages arising from the use of our services.
8. Compliance with Laws
All services are conducted in compliance with applicable regulations, including Department of Transportation (DOT) guidelines where applicable.
9. Changes to Terms
We reserve the right to modify these Terms & Conditions at any time. Continued use of our services constitutes acceptance of any updates.
